Is it possible to over drive a speaker and damage it without actully hearing distortion?

I don't think so although I'm not sure. Overdriving a speaker also involves over excursion of the driver which should produce audable results.

I believe it is possible. The mechanism of failure is heat buildup that burns the VC, and there is no reason why this can't happen with no audio distortion. In fact, it is pretty easy to fry tweeters with about 20 volts of DC which would make no sound at all.
